  • Alcohol, substance abuse, and treatment: Treatment for alcohol addiction, abuse of substances, and treatment of resulting conditions is excluded from coverage under this policy.
  • Refractive error: The treatment of refractive errors below the defined threshold is excluded from coverage.
  • Unproven treatments: Unrecognized treatments and therapies that have not been proven to work are excluded from coverage.
  • Infertility, contraception, sterilization, and surrogacy: Any treatment for infertility, including IVF, contraception, sterilization, surrogacy, and fertility treatments, is excluded from coverage.
